We are seeking a highly organised and clinically capable GP Assistant and Nurse to support General Practitioners with a mix of clinical coordination, medical documentation, patient follow-up, stakeholder liaison, and professional administrative support.
This is a unique hybrid role suited to a nurse who enjoys working across both patient care and high-quality documentation. The role includes work-from-home duties as well as in-person work at the practice, residential aged care facilities, and mass immunisation clinics in RACFs.
The position is expected to be 25–30 hours per week, with hours designed to fit largely within school hours.
You will work closely with GPs to help improve patient care, streamline clinical workflows, and support proactive management of patients with chronic health conditions, particularly those living in residential aged care facilities.

Experience with residential aged care facility patients, health assessments, care plans, chronic disease management, immunisation clinics, or GP-led aged care services will be highly regarded.
