About the Program
The Wesley Care finder case worker plays a vital role in supporting vulnerable older people who require intensive assistance to navigate and access aged care and related supports.

We provide support to connect participants to relevant programs and services in the community, targeting individuals who face barriers such as:

  • Isolation or lack of a support person.
  • Communication challenges, including literacy barriers.
  • Difficulty processing information or making decisions.
  • Hesitancy to engage with aged care services or institutions.

About the role

The Wesley Care finder supports vulnerable older people who are unable to navigate the aged care system on their own and who do not have someone else to assist them. This is a frontline, community‑based role focused on ensuring older people do not miss out on the care and supports they need.

Working within a person‑centred, outreach‑based model, Care finders provide free, intensive, face‑to‑face support to help clients understand their aged care options, connect with My Aged Care, arrange assessments, choose appropriate providers, and link with community and support services. The role involves building trusted relationships and supporting people experiencing complex barriers such as social isolation, health concerns, cognitive decline, financial hardship, or cultural and language challenges.

Regular travel within office hours is a significant part of this role, with the Case Worker working across the local PHN region to meet clients in their homes and community settings.

Essential skills/knowledge

  • Remuneration Diploma or higher in a relevant field (e.g., aged care, community services, social work, allied health, nursing)
  • Proven experience supporting older people, people with disabilities, or individuals experiencing social disadvantage using person-centred, strengths based practice.‑centred, strengths‑based practice.

  • Ability to identify client needs, provide appropriate service recommendations, and navigate multiple service systems.

  • Demonstrated commitment to supporting people from diverse backgrounds, including Aboriginal and Torres Strait Islander peoples, Culturally and Linguistically Diverse communities, LGBTIQA+ communities, and rural/remote populations.

  • Proven ability to build and maintain strong relationships across service providers, networks, and community organisations.

  • Strong problem-solving skills, including the ability to assess issues, identify solutions, and take appropriate action within role boundaries.

Renumeration

Salary and employment conditions are in accordance with the Social, Community, Home Care and Disability Services Industry Award 2010, Level 5, with capacity for salary packaging opportunities and the provision of a fully maintained motor vehicle available for private use.
